Launched in March 2010, the Medication Management Center (MMC) at the University of Florida’s College of Pharmacy connects with WellCare Medicare Part D members who meet eligibility requirements for MTM services. Thousands of WellCare members benefit from MMC’s faculty, clinical pharmacists and student pharmacists providing MTM services.
AvMed Health Plans, Inc.
The Medication Management Center formed a new partnership with AvMed Health Plans to deliver clinical intervention programs designed to impact Star Ratings pertaining to CMS Patient Safety Measures, HEDIS Measures, as well as other quality measures. In addition, the MMC will be expanding their role by providing Medication Therapy Management services to AvMed’s Medicare Part D members who meet eligibility requirements in 2014.
Medicaid for the Aged and Disabled (MEDS-AD) Waiver Program
The Medication Management Center formed a new partnership with Florida Medicaid and the Agency for Health Care Administration to provide Medication Therapy Management services to applicable Florida Medicaid patients under the MEDS-AD Waiver Program. This partnership allows the MMC to expand its reach to serve a unique population in Florida.
The Nontuberculous Mycobacteria Clinic at UF Health has partnered with the UF Medication Management Center to provide Medication Therapy Management to this unique population of patients.This collaboration provides pharmacy expertise at this clinical practice thus decreasing medication related problems. In addition, this partnership provides a venue of research into how MTM can improve patients’ behaviors and perceptions of their medications and disease state.
Alachua County Community Support Services
The UF Medication Management Center and the Alachua County Crisis Center have teamed up to provide rotation students and center employees with crisis training. This 2+ hour training session is presented by Alachua County Crisis Center personnel. Students and employees are trained to identify potential suicide risks, provide empathy, and identify call escalation scenarios.
The Alachua County Crisis Center offers 24-hr/day phone crisis and suicide intervention counseling to all residents of Alachua County. In addition, they provide face-to-face counseling, have available a 24-hour emergency mobile outreach team, and offer training on a variety of topics throughout the community.
